The Telephone Triage Practitioner will manage calls from individuals who are experiencing a high level of mental health distress, their families and professionals. The role will work in close partnership with the NHS Crisis Team acting as a link between individual, professional and other services by ensuring continuity of support by delivering an efficient and effective person-centred service.
The Practitioner role will navigate the individuals through defined pathways to enable delivery of the right help at the right time with the right outcome. In addition the practitioner will be expected to undertake administrative duties.
The 37.5 hours will be worked over 3 shifts on a rota basis. The triage service operates 24/7. Shifts can be days only, nights only or a mix of both. Please state your preference on your application.
